The entire team at Aguiar Plastic Surgery & Medical Spa wants to make your experience the best it can be.  Our goal is provide you with the safest and most effective procedures and services.  In order to do this, a full comprehensive medical history is required.  We appreciate your understanding and cooperation.

New Patients

In an effort to maintain efficiency in the office, we ask that new patients fill out the required paperwork before their appointment.  For you convenience, you have two options.

1) Access our patient portal: On the patient portal you will be filling out your personal information (ie. Name, address, phone) as well as 3 medical questionnaires.  If you have any questions regarding the patient portal, please call the office and we will be happy to help you.

2) You may download, print and complete the “New Patient Packet”.  We ask that this be completed prior to your appointment.  If possible, please fax in your completed packet to 813-739-0917.

Financial Policy and Cancellation Policy

Payment is due at time of service unless you have health insurance plan in which physician participates; for those patients, copays, deductibles, and coinsurance will be collected.  You will be responsible for any balance not covered by insurance.  You are responsible for knowing your deductible and whether or not it has been met.  We reserve the right to discharge patients for frequent no-shows and last minute cancellations.

Insurance

Currently, Dr. Aguiar participates in the following programs. Insurance contracts are frequently reviewed and changed. If you have questions about your specific insurance plan, we will be happy to do a complimentary benefits check prior to your appointment. 

*If you do not see your insurance plan here, you may still be eligible to utilize “out-of-network” benefits.

Please note that we cannot accept Medicaid even if it is a secondary insurance.
